BECOMING A CLIENT

Making an Appointment

To begin services with us, please fill out the forms below. Once completed,  our office will call or text to schedule your first appointment - which usually can be scheduled within a week. 

Occasionally, we may determine that our services do not match your needs. In that case,​ we will refer you to an appropriate provider.

New Client Intake Form

​First Appointment: Diagnostic Assessment

This process consists of gathering detailed information to assess an individual's behavioral health needs to determine the best service mix of mental health and rehabilitation services for improvement and recovery.

The evaluation is designed to: diagnose 
mental health conditions such as anxiety, depression, schizophrenia, postnatal depression, eating disorders and psychotic illnesses and to differentiate between mental and physical health problems.

Second Appointment: ​Psychiatric Evaluation

This process will review:​
  - Reason(s) for your appointment
  - Psychiatric and social history
  - Medical history including medication history
  - Information about other services being received

​Individualized Service/Treatment Plan

Once the psychiatric evaluation and diagnostic assessment have been completed, a CBHC counselor will work with you to create your Individualized Service Plan (ISP).  

Your ISP will include:
  • High-priority treatment goals
  • Measurable objectives
  • A timeline for treatment progress
  • Agreement on how to track progress

Setting goals helps Clients:
  • ​​Set priorities
  • Avoid feeling overwhelmed
  • Achieve more
  • Stay motivated
  • Feel satisfied
  • Boost self-confidence
  • Concentrate better

Ongoing Appointments

CBHC providers follow a problem-solving approach to therapy and treatment, which includes comprehensive evaluation, collaborative development of goals, and psychiatric assessment as well as medication when appropriate.

Treatment and therapy for older adults may focus on the transitions of aging including coping with bereavement or loss, family support, memory loss or dealing with health issues and  independent living skills.


A Qualified Mental Health Specialist (Case Manager) may be assigned to help individuals on a regular basis to access needed services and to improve daily living skills with respect to work, family and integration within the community.
